Congratulations on your interest in hosting an event for the Lone Star Miata Club! Below are essential documents needed to ensure proper planning and liability coverage.

• To host an event, first please fill out the LSMC Event Planning Template (PDF) and send to Secretary@lsmiata.org .

Prior to day of run please ensure you print out the applicable waiver form to be signed based on driving event type below:

• To participate in a run/street driving event, a Rally Release and Waiver of Liability and Assumption of Risk Agreement (PDF) must be completed.
• To participate in a track event, a Release and Waiver of Liability, Assumption of Risk and Indemnity Agreement (PDF) must be completed.
• For a Minor to participate in ANY event, a Parental Consent, Release and Waiver of Liability, Assumption of Risk, and Indemnity Agreement (PDF) must be completed along with the second page, Minor’s Assumption of Risk Acknowledgement.

REMINDER If you are the event coordinator, please download a copy of all appropriate forms, print several, and take them with you to the driver and navigator meeting. Be sure and have each participant sign a line on the waiver.

Completed forms go to Secretary or President upon completion of event.
